Positive Covid Case Submission Form District

In an effort to expedite the Covid-19 contact tracing process, as well as gather information from parents and staff members, South Huntington School District has recently created a Google survey that allows individuals or families infected with the covid-19 virus to provide updated information of a positive result of a student/staff member in a quick and easy manner.

This information will allow the school district to further contact and evaluate whether or not other individuals within the school building need to quarantine. This will also lessen the amount of time needed for questions that must be asked by a School District representative with regards to information needed by the Suffolk County Department of Health.

Please see the link below or visit the district website’s main page to input any information of a positive covid case. This form does not replace a parent's ability to communicate directly with their child’s principal or for a staff member to communicate directly with their supervisor.
